Well over a million Americans are injured each year by drug errors. A medication error is defined by the National Coordinating Council for Medication Error Reporting and Prevention as “any preventable event that may cause or lead to inappropriate medication use or patient harm while the medication is in the control of the health care professional, patient, or consumer.” Drug errors include wrong dosage, overdose, drug interactions and medication mix-ups. According to a study by the U.S. Food and Drug Administration (FDA), the most prevalent medication errors include the following:
- Improper dose of medicine – 41%
- Giving the wrong medication – 16%
- Administering the drug incorrectly – 16%
